It's horrible - basically bullying and a freak show. Half the guys are strung out on heroin (I saw one episode where one of them threw up after hyping up in between rounds). That was enough to put me off. It's preying upon the vulnerable.
Why do they get away with it? Because it's America, it's tv, they're (almost exclusively) gay, and therefore it's ok to put them in a modern day version of the stocks.
