Generally the whole aim with torture is to carry out and have the word spread to people you want to hear but without anyone being able to prove you did it. On that basis they've been reasonably successful so far.
Quite possible that they didn't want to kill him but someone unskilled nicked an artery whilst trying to just cause some pain.
